New Operational Alerts: Forklift Tip Over, Fire Incidents & The Importance of Proper Communication, & Hazards of Scaffolding Board Uplift

Operational Alert’s, inform Signal Mutual Members of serious incidents or a significant near-miss within the Mutual. If you have similar operations, please share this Operational Alert with operation managers, supervisors, equipment operators, and all affected employees. Forklift Tip Over

A Forklift operator was driving in reverse inside the paper warehouse after discovering a mechanical issue that left their mast stuck in an elevated position. The operator was taking the equipment to the shop to report the issue when they drove through a roll-up door with the mast elevated higher than the top of the door frame, causing the lift to flip forward. 

Fire Incidents & The Importance of Proper Communication

In the ship repair industry, marine chemists play a crucial role by ensuring that hot work is carried out safely. This places a significant responsibility on those who communicate with the chemist to fully understand the work they are requesting.

Hazards of Scaffolding Board Uplift

A near-miss incident occurred on a recent aircraft carrier project when workers were staging a mast tower built by a subcontractor.  A steel mesh scaffold plank was uplifted from the deck of a rest platform and fell the span of four decks before landing in an active work area on the flight deck. Fortunately, the area below was clear, and no injuries occurred.
