3 New Operational Alerts: Climbing For Module Access; Container Falls On Top Handler Cab & Did You Have Training On That New Equipment?

Climbing for Module Access

A shipyard worker needed to gain access to a raised platform just under 6’ high to get onto the module where work was to be performed. Stair and ladder access had been set up on the opposite side of the module for access, but the side the employee was attempting to access had not yet been configured with proper fall restraint.

Container Falls on Top Handler cab

A top handler operator was seriously injured when a six-high block of empties toppled over, and a container landed on the top handler cab. The operator was hoisting the last can from the stack in front when the spreader caught the bottom of a three-high container in the adjacent stack.

Did You Have Training on That New Equipment?

A new order picker forklift model was delivered to a Members warehouse and the warehouse receiver was given brief operating instructions from the delivery driver. Before formalized training was accomplished by the Member company, the warehouse worker began operating the new forklift model.
