It is imperative that management, at all levels review their operations for similar hazardous conditions, acts, and their controls.
A reach stacker tip-over incident while operating the equipment along a roadway adjacent to a stack of shipping containers. The root causes were identified as excessive operating speed and an overextended boom. These cumulative factors compromised the machine’s center of gravity and led to a forward loss of stability.
